Juxtapapillary duodenal diverticula and their possible complications are not frequent findings. We present the case of a woman with a giant juxtapapillary diverticulum, complicated by diverticulitis and areas of perforation of the wall that required urgent surgical treatment. We present the preoperative findings on computed tomography and magnetic resonance imaging.

Fulminant infection with emphysematous changes in the biliary tract and air-filled liver abscesses.

Gas-producing infections are related to anaerobic bacteria and usually associated with immunosuppressive pathologies such as diabetes and have a fulminating course. We present the clinical course of a patient with diabetes whose infection progressed rapidly and ended in her dramatic death in a short period; surgical therapy was impossible, and the disease behaved similarly to type I emphysematous pyelonephritis.

Idiopathic enteroenteric intussusceptions in adults.

BACKGROUND: Intestinal intussusception is infrequent in the adult, especially in the idiopathic and transitory forms. METHODS: We reviewed the medical records and imaging of four patients with seven idiopathic and transitory intestinal intussusceptions discovered in the course of barium meal and small bowel follow-through radiologic studies carried out for nonspecific abdominal pain. RESULTS: The patients had nonobstructive intestinal intussusception, with no demonstrable underlying cause. Some intussusceptions were resolved by applying pressure to separate the pathologic intestinal loop. No complications associated with the intussusceptions were found during subsequent follow-up. CONCLUSION: There are cases of idiopathic nonobstructive intestinal intussusception in the adult, with few or no clinical repercussions that require aggressive medical intervention.

Spontaneous pneumatocele and pneumocephalus associated with mastoid hyperpneumatization.

We present a case of spontaneous pneumatocele and pneumocephalus associated with mastoid hyperpneumatization. Collections of air such as these are normally due to injury or surgery, and more rarely, infection or tumor; exceptionally they may be spontaneous. The patient in question complained of a progressively enlarging retroauricular mass which increased in size on Valsalva maneuvers; he had no other symptoms. The etiology, skull X-rays, computed tomography and follow-up after surgery are shown here.

Acute suppurative thyroiditis secondary to piriform sinus fistula: a case report.

We present a typical case of acute suppurative thyroiditis (AST), associated with a piriform sinus fistula. We illustrate the case with an ultrasound picture of a hypoechogenic perithyroid mass and a CT scan using intravenous iodine contrast showing a perithyroid hypodense mass with peripheral enhancement, the mass also having intrathyroid involvement. When clinical and radiological findings suggest the presence of AST, it is necessary to rule out the presence of a piriform sinus fistula by means of a radiological study with barium contrast. Piriform sinus fistula is a rare abnormality derived from the branchial arch which is directly related to recurrent episodes of AST; surgical excision is hence, necessary to avoid such episodes.
